Thank you for considering participation in our on-line Customer Satisfaction survey.

Your feedback is very important to us. It will help us to identify areas where we can improve services and better understand the needs of our users.

If you agree to participate in this survey you will be asked questions about your use of the Library and your satisfaction with the Library’s services, collections, facilities and policies. You will not be asked to provide any personal indentifying information except for an optional question for your email address and postal code which will be used for a reminder for the annual survey follow up and geographic mapping of our users. Participation in this survey should take approximately 7 - 10 minutes for the regular survey and 15 minutes for the in-depth survey.

You may decline to answer particular questions if you wish, and you can withdraw from participation at any time. Declining to answer or withdrawing your participation will have no impact on your relationship with McMaster University or the Library. Participation in this survey is voluntary and the final decision to participate is yours. Your information will be obtained anonymously and will be stored on a secure server in Canada.
